How to Refresh a PDF Form

hi,
I have a PDF form, put it in a Web server.
I use this form to fill some clients info, so I demand it a lot.
Every time an employee fills a new Form, the Form stays with the old data, even if the Refresh button of the web browser is pressed.
Does be a mode to refresh the class (clear all the data, checkboxes, etc) when the employee press the Refresh button of the web Browser.
